Why decisions get re-litigated

Commercial projects have 5-8 standing meeting cadences: weekly OAC, weekly GC-sub coordination, bi-weekly MEP coordination, monthly owner check-in, ad-hoc design-team reviews. Each generates minutes. Each set of minutes contains decisions that affect the project.

Three weeks later, on a different day, in a different context, the same question comes up. Did we decide polished-concrete or LVT in the elevator lobby? The minutes say so — if you can find them. Most teams can’t, so the decision gets re-litigated, sometimes with the opposite outcome.
